Instructions for School-Age Services:
The School-Age Referral Request Form is the key document used in making referrals for services provided by the CAIU. This form lists the services, evaluations, and placements available, with the necessary documentation required to accompany a Referral Request listed on the second page. The Referral Request Form can be obtained by clicking on the link below.
In some instances, additional specific forms are noted as required documentation for referral. These forms are available in the Central Referral Packet or can be obtained by contacting the person listed at the bottom of each service’s profile.
Completed and signed Referral Requests, along with any required accompanying documents, should be submitted to the Central Referral Secretary at the CAIU Summerdale Office. Final documentation for requested related services, support to school personnel or placement is required prior to the start of service.
